An email will be sent out at the end of each quarter as a reminder. It will contain a link to the form and the instructions for filling and turning in the form. If you do not receive the email there is no need to worry, as it is only a reminder and email is terribly unreliable. Please DO NOT rely on the reminder email to report. Instead, please set reminders for yourself of the due dates and use the password protected page for reporting.
**Only fill out one form per student per quarter.
After reporting, you should have received 2 emails, one from me & one from Google Forms. The Google email has an "Edit Response" link in it. Please use that link to correct your response.
If you've made a mistake and can't submit a change to your already submitted form, please email me to let me know, DO NOT submit a new form.
Submitting the attendance report quarterly is mandatory for enrollment. Attendance reports are due at the end of each quarter for the school year to date.
Please fill out even if you are new and have little to no attendance to report, as this is how I keep track of currently enrolled students and report to the state my grade level enrollment. You may use this attendance form for your own records and to make filling the online form easier.
Please include any and all attendance for the school year to date, from any source, and notate so on the form. All attendance for the school year counts, even if it were from homeschooling or another school. To be accepted into this group you must send me a message via our regular Facebook page with your email address and your child’s name and date of birth. Once I’ve established that you are currently enrolled I will approve your request to join. This is for the privacy and safety of our families.
Please remember that the quarterly attendance counts are cumulative. This means that each time you report you are not only reporting the count for the quarter, but for all quarters since the beginning of your school year from any source. Example: Assuming you’ll report every single day (which you may if you’d like) and your school year begins on Sept 1, 2015. The attendance due beginning Sept 1st (ending Aug 31st) will be 0 (zero) days entered, if you are a first time student. (If you’ve been enrolled for more than one school year then you would never enter a zero count, the Sept 1st would always be 365 or whatever your year end total would be.) The next quarter due beginning Dec 1st (ending Nov 30th) will be a total of 90 days. The next quarter beginning March 1st (ending Feb 28) will be a total of 180 days. The next quarter beginning June 1st (ending May 31) will be 273. The last and final quarter due for your school year, due beginning again on Sept 1st (ending Aug 31st), your total will be 365.
If you need to report attendance to finish up your year, but have already begun a new year before time to report, still please only fill out one form. You may put your year ending number and report your new year on the next report or if you've previously reported at least 180 on another form you may start over. ie. You ended your year with 180 days on July 1st and began your new year on August 1. For the reporting period ending Aug 31st you could either put 180 again or you could put your new year's total. Either way is fine as I only go back to the most recent attendance form for your year to date total.

Florida law requires at least 180 days of regular (consistent) attendance annually. Your year begins the day you enroll and ends 365 days later. However, just as if your child were transferring from one public school to the next, any attendance accumulated during the current school year for which you are enrolling will count toward your 180 days. It is the parent’s responsibility to send in attendance. It is the legal requirement to remain in compliance with Florida’s compulsory attendance law. You will be sent an email reminding you when your attendance report is due. The link will be posted on social media and on the password protected page on this website. The form will remain open for one month, but please turn it in as soon as possible when due. This is for your protection! If I am contacted by authorities I need to have the information readily available to supply them so that you remain in compliance and out of trouble!
